The College of Dental Medicine (CDM)
is pleased to offer Columbia Dental Plus Plan, a comprehensive
dental package. Employees selecting this option
must utilize one of the faculty practice sites listed below
or another Columbia Dental Plus Plan Provider to receive the
maximum benefits for this plan.
The
Plan has no deductibles, no exclusions and no limitations
for pre- existing conditions. The benefit includes all preventive
and routine restorative services covered at 100% with all
other procedures covered @ 50% up to $2,000. maximum*. The
patient is responsible for the payment of the 50% not paid
by the plan.
*The
total cost of orthodontic braces are not included
in the $2,000 annual maximum and there is a separate lifetime
orthodontic maximum of $2,000.
Teachers College has a $1500. Orthodontic lifetime maximum. Once you have exhausted your annual maximum, you will still receive a 25% discount from your dentist
